(If you want to be listed in additional categories, register again on a new form, select a new category, and pay an additional $65 (returning participant)/$70 (new participant).) For each category, you must be the actual producer/craftsperson/owner who makes or creates the product or provides the service.
NOTE: Category selection will be approved by the NH Open Doors committee and the final list may be edited prior to being posted on the NH Open Doors web site. |

Agreement:
I understand that participation in the NH Open Doors event is subject to approval by the two partnership organizations, New Hampshire Made and the League of New Hampshire Craftsmen. By participating in the NH Open Doors event, I am agreeing to the following:
TO BE OPEN BOTH DAYS OF THE NH OPEN DOORS EVENT, November 6 - 7, during the hours I indicated on this form;
TO PROVIDE THE ACTIVITY / ATTRACTION / SALE indicated on this form on both days of the event;
TO PICK UP MY PROMOTIONAL MATERIALS AT THE LOCATION SPECIFIED no later than one week prior to the event and post them in visible locations on both days of the event;
TO PROVIDE, IF POSSIBLE, A RECIPROCAL LINK TO THE WWW.NHOPENDOORS.COM SITE from my own business web site in order to help promote the event and generate additional traffic to my own site; TO COMPLETE AND SUBMIT THE ONLINE PARTICIPANT SURVEY no later than two weeks after the event;
TO ASSURE CUSTOMER SATISFACTION by greeting visitors warmly, providing information about my business, and responding courteously to all questions;
TO UNDERSTAND THAT MY $65/$70 PARTICIPATION FEE IS NON-REFUNDABLE once I have been approved and posted on the www.nhopendoors.com web site for any length of time. |

If you choose to pay by check, a copy of this completed application will be sent to the e-mail you provided above with instructions for sending your payment to League of NH Craftsmen, 205 North Main Street, Concord, NH 03301.
